Mandatory Requisites for an In-Clinic Appointment
Please pre-book your appointment to avoid disappointment
Willingness to co-operate with clinic guidelines
Only 1 attendant to accompany the patient
Covid-19 Screening consent form to be filled and signed
Wear a MASK at all times whilst in the clinic
Maintain social distancing as per seating arrangements
Use sanitizer and / or soap wash as requested by staff
Avoid files/bags. Share relevant reports online in advance
Please avoid attending the clinic if you have fever or cough - Digital/ Card payments preferred
Reduce your chair time and contact in the clinic premises
